Employee dies from head injuries after falling from van

At approximately 10:45 a.m. on October 4, 1994, Employee #1 was riding near the open right side sliding door of a 1981 GMC van, where the seat had been removed. Employee #1 fell from the van and received head injuries, for which he was hospitalized. He later died.
