Employee dies in fall from elevated platform

Employee #1 was installing electrical fixtures outdoors at the entrance of a mall that was being renovated. Employee #1 was using a borrowed tradesman scissor lift, model 25-48E, to elevate himself 25 feet to reach the fixtures. He fell from the platform, struck the concrete pavement, and died. What caused him to fall is unknown; however, the scissor lift did not have an appropriate height guardrail. The scissor lift was on level ground. The lift was owned by a different employer, who was preparing to remove it from the site. Employee #1 borrowed the lift without permission. Other than missing the top guardrail, the scissor lift was in good working order.
