Employee is run over and killed by forklift

At around 11:20 a.m. on or about October 20, 1994, a forklift operator was hauling a dumpster that had just been dumped into a trash container and Employee #1 was walking next to a manufactured mobile home. The operator was transporting the load in front of the forklift at approximately 6 inches off the ground. The trash container measured approximately 4 feet 5 1/2 inches in height and was 4 feet wide and 6 feet long. Employee #1 was struck by the trash container, then run over by the forklift. She died from her injuries.
