Investigation Summary
At approximately 9:30 a.m. on October 30, 1992, Employees #1 and #2 were preparing to scrape paint from an overhead canopy. The employees were working from a mobile scaffold that was 30 in. wide, and planked out with two 2 in. by 10 in. scaffold grade wood planks. They were working from the top level of two frames at a height of 10 ft 4 in. when the scaffold tipped over and fell, sending Employees #1 and #2 to the concrete pavement. Employee #1 died of internal injuries; Employee #2 suffered contusions. He was treated and released from the hospital.
