Employee killed in fall from roof or ladder

Employee #1 was assigned to remove an old sheet metal roof and to install a new one. He was last seen carrying an ax and preparing to ascend a metal ladder to the roof. Coworkers working in the area heard a crash and turned around to see Employee #1 lying dead on a concrete pad. It is not known whether Employee #1 fell from the roof or the ladder. The investigation revealed that Employee #1 did not use any fall protection while on the roof. The roof's height varied from 12 ft to 19 ft. The ladder used was not secured, and did not extend at least 3 ft beyond the roof. The top section of the extension ladder was used as the bottom section.
