EMPLOYEE INJURED IN FALL THROUGH UNGUARDED FLOOR OPENING

TWO EMPLOYEES WERE INSTRUCTED TO FIND SOME SHEET METAL TO REPAIR PANELS ON A DAMAGED MOBILE K-MAG. THEY LOADED THE METAL ONTO THEIR PICKUP TRUCK, BUT THE TRUCK WOULD NOT START. THEY DECIDED TO WALK FOR ASSISTANCE, TRAVELLING THROUGH AN OLD BLOOMING MILL BUILDING. EQUIPMENT HAD BEEN REMOVED FROM THE MILL'S MOTOR CONTROL ROOM SO THAT UNGUARDED FLOOR OPENINGS TO THE BASEMENT EXISTED. ONE EMPLOYEE WALKED IN THE AISLEWAY. THE OTHER (EMPLOYEE #1) STRAYED FROM THE AISLEWAY AND STEPPED INTO A FLOOR OPENING. THE OPENING WAS 5 1/2 FEET WIDE AND 10 1/2 FEET LONG. THE EMPLOYEE FELL ABOUT 13 FEET, RECEIVING SERIOUS HEAD INJURIES. ALTHOUGH THE VEHICLE ACCESS GATE TO THE MILL BUILDING WAS LOCKED, OTHER DOORS INTO THE UNUSED BUILDING WERE OPEN AND THE FLOOR OPENINGS WERE UNGUARDED.
