Investigation Summary
Employees were removing partition doors used to section a gym in two. After the employees lowered the 16th or 17th door to the floor, the scaffold tipped as the C-clamps were raised to the top. Two employees were dropped to the ground. Employee #1 was killed, and Employee #2 was hospitalized.
