Employee crushed to death by falling barrier

Employee #1 was patching concrete preformed barriers while a material handler was placing them, with a forklift, on the floor to be patched. As a barrier was set down, it fell over and struck another barrier, pinning Employee #1 between the two barriers. Employee #1 died from his injuries.
