FALL FROM LADDER
An electric utility employee was performing maintenance work on an expansion joint on a 2.1-meter-diameter water line. The employee was working from a ladder with her back to the ladder. She fell from the ladder, apparently while descending. She fell a distance of 6.1 meters from the ladder into a flume. She died of her injuries.
