ELECTRIC SHOCK - DIRECT CONTACT WITH ENERGIZED PARTS

TWO EMPLOYEES HAD GONE TO A LARGE OIL PRODUCTION LEASE TO REPLACE A FUSE IN A CUTOUT SWITCH. THE SWITCH WAS ON A UTILITY POLE AT A PRODUCING OIL WELL. WHILE THEY WERE REPLACING THE FUSE, THE FUSE HOLDER FELL AND BROKE. THEY HAD NO REPLACEMENT, SO THEY WENT TO THEIR EMPLOYER'S RESIDENCE TO GET ANOTHER. THE EMPLOYER TOLD THEM TO RETURN TO THE WORK LOCATION AND TAKE A SIMILAR FUSE HOLDER FROM ANOTHER PLACE AND USE IT AS A REPLACEMENT. THEY RETURNED TO THE WORK LOCATION AND LOOKED FOR A SIMILAR FUSEHOLDER. THEY COULD FIND NONE. ONE OF THE TWO EMPLOYEES, WHO WAS THE SUPERVISOR, SENT HIS COWORKER TO THE TRUCK WITH THE HOT STICK. SHORTLY THEREAFTER, THE COWORKER HEARD A ZAP AND A SCREAM. HE FOUND THE SUPERVISOR INSIDE A SMALL FENCED AREA CONTAINING 7620-VOLT ELECTRIC EQUIPMENT. THE SUPERVISOR HAD BEEN ELECTROCUTED.
