Employee dies of carbon monoxide poisoning

After midnight Employee #1 was doing janitorial work in the front room of an office building. The boiler tubes on the Ray Pak Model 1330T heating boiler had not been cleaned in some time and had become sooted over and clogged up, preventing the heating element from venting properly, As a result, the burner produced carbon monoxide, which entered the heating air system, spread throughout the building. and killed Employee #1. He was found in the front room of the building at the beginning of the next work day by office personnel.
