Employee crushed to death under falling hopper

At approximately 8:45 a.m. on November 10, 1994, Employee #1 was sacking coke briquettes at a carbon plant. The briquettes were an experimental process conducted by the company to determine the market for this form of coke. The hopper that was used to fill super sacks with the briquettes was 10 feet 7 inches high and the box was 52 inches by 72 inches at the top and 72 inches deep with a 12 inch filler neck. The hopper was approximately one half full (the amount accumulated between removing a full sack and replacing it with an empty sack) and weighed approximately 1,000 pounds. A forklift was used to hold the sacks in place while they were being filled, and to remove the full sacks and replace them with empty sacks. The hopper support was a makeshift unit constructed with inadequate footing so that it could easily tilt and be toppled if struck by the forklift. The hopper fell on Employee #1 and killed him.
