Employee killed when crushed by elevator

Employee #1 was disengaging the door locking mechanism from inside an elevator shaft so that a coworker could spray pesticide under the elevator door at the first floor. Employee #1 entered the shaft from an adjacent elevator which he did not secure before entering the shaft. That elevator subsequently struck Employee #1 as it was moving downward. He died.
