Investigation Summary

Investigation Nr: 758482
Event: 09/24/1990
Employee struck and killed by falling I-beam

Two employees were standing on the ground approximately 5 ft from the structural steel supports of a main bridge tower discussing their next move as they replaced or repaired beams and columns on the bridge tower. Without their knowledge, the Inland Steel Company's coke plant pusher car, which was traveling north with its leveling arm extended beyond its safe limit, struck the temporary falsework supports and caused several pieces of pipe and an 8 in. wide, 12 ft long I-beam to fall 20 ft to the ground. The beam struck Employee #1 on the head and fractured his skull. He died.
