Investigation Summary
At approximately 6:00 p.m. on August 26, 1991, Employee #1 and a coworker were using a 5,000 lb rated capacity fork truck to lift and reposition a 60 ft long steel beam that weighed approximately 6,000 lb. The fork truck turned over sideways and the steel beam fell onto Employee #1's feet, causing serious injuries. He was transported to Union Hospital in Terre Haute, IN.
