Employee's fingertip is cracked in power press

At 1:30 p.m. on October 29, 1990, Employee #1, a laborer, was hand feeding 20 inch long steel strapping into the die area of an L&J 60-ton full revolution mechanical power press. The press, which is foot pedal operated, had no point of operation guarding. Employee #1's right index finger entered the die area and was pinched by the die closure. The bone of his fingertip was cracked. Employee #1 was treated by a local physician and released for work; he lost no work days.
