Employee's fingers amputated in press

At approximately 8:50 a.m. on September 29, 1990, Employee #1 was placing a part into the point of operation on the 27 ton L and J press. A control arm came apart, causing the press to enter continuous run mode and catch the employee's left index and middle fingers. Both fingers were amputated. Employee #1 was sent by ambulance to the hospital, where she was treated and released.
