Employee killed when butane lighters in pocket ignite

Employee #1 was cutting seized bolts when a red-hot piece of slag flew into his pocket and melted through one or two plastic disposable cigarette lighters that contained butane. The butane ignited, burning Employee #1's clothing and body. He died.
