Investigation Summary

Investigation Nr: 14302160
Event: 12/17/1990
Employee killed when caught in machinery

Employee #1, a temporary laborer, was assigned to clean a fiberboard production line with compressed air. He approached a precompressor from the west side of the line, then crossed a chain barrier that was parallel to the machine to blow off wood fiber that had accumulated under the machine's return belts and rollers. The employee then crawled through a 21 inch high opening between a horizontal beam and a steel base platform 20 1/2 inches above the floor. The precompressor was temporarily shut down for seven minutes. Employee #1's body was discovered on the steel base platform. Three minutes after the precompressor was started up, his body passed between the return belt and a 13 inch diameter steel roller. The employee died of massive, multiple injuries. He had no training in energy control procedures.
