Investigation Summary

Investigation Nr: 785543
Event: 02/25/1991
Employee suffers fatal heart attack during training exercise

At approximately 3:00 p.m. on February 25, 1991, Employee #1, of RLK & Company at Timlodge, Government Camp, OR, was participating in a fire drill. He donned turnouts and a SCBA with the help of a coworker. The hose to the mask's regulator was not connected because there was not a live fire. Employee #1 and his partner, who was not wearing SCBA and gear, then walked upstairs and approximately 50 feet down a hallway. At that point, Employee #1 felt faint and sat down on the stairs to catch his breath. Approximately five EMTs came and evaluated Employee #1. He was transported by ambulance to Mt. Hood Medical, where he was treated for a possible heart attack. While in the hospital, Employee #1 could not be revived. He died between 7:00 and 8:00 p.m. The medical examiner's office in Portland is conducting an investigation.
