Investigation Summary

Investigation Nr: 170128789
Event: 12/20/1990
Employee injured when struck by falling metal tube

At approximately 11:30 a.m. on December 17, 1990, employees of Little Iron Works were preparing to off-load thin gauge steel materials from a vendor's truck. Employee #1 and a coworker placed a 4 in. by 4 in. by 16 ft 8 in. metal tube on the truck bed as a stiffener for the thin gauge materials that were to be unloaded. Employee #1 put his end of the tube on the truck bed and walked over to the coworker. Employee #1 was moving sideways as he maneuvered the tube when he tripped, fell to the pavement, and was struck by the falling tube. Company personnel administered first aid until EMTs arrived and transported him by ambulance to Salem Memorial Hospital.
