Investigation Summary
AN 18-YEAR-OLD GENERAL LABORER FOR A SAND AND GRAVEL FIRM WAS CLEANING GRAVEL FROM UNDER AND AROUND CONVEYOR SYSTEMS FOR A CEMENT-TREATED BASE PLANT. HE ALSO LOOSENED CEMENT PRODUCT IN A HOPPER AS NEEDED. THE EMPLOYEE WAS FOUND DEAD ABOUT 15 FEET FROM A CONVEYOR. THERE WERE NO WITNESSES TO THE ACCIDENT; HOWEVER, EVIDENCE (ELECTRIC ARCING) POINTED TO ELECTRIC SHOCK AS A POSSIBLE CAUSE OF DEATH.
