Investigation Summary
At 11:50 a.m. on June 30, 1991, Employee #1 was kneeling over the broke hole pipe guard when the reel spool pipe brake began to descend. Coworkers tried to warn him, but were too late. The pipe brake descended, crushing Employee #1's head against the broke hole pipe guard. An emergency rescue was made and the Employee #1 was transported to the Navapache Hospital, where he was pronounced dead.
