Event: 04/15/1991
Employee dies of heart attack

At approximately 7:40 a.m., Employee #1 was working on the shredder scrap handling magnet of a P & H 25 ton #3 crane. He was attempting to lengthen the power cable to the magnet by loosening the bolts that clamp the cable to the pulley. As he was loosening the bolts, the crane operator observed the employee stop and drop his arms. He then clutched his chest, made a slight turn to the left, and fell head first to the ground. Emergency services responded and transported the employee to Scottsdale Memorial Hospital, where he was pronounced dead. The medical examiner later determined that Employee #1 had died of a heart attack.
