Event: 01/21/1991
One employee killed, three injured in caisson cave-in

At approximately 10:53 a.m. on January 21, 1991, Employees #1 and #2 were drilling a caisson for a highway bridge when the caisson caved in. Employee #1 was trapped and died of asphyxia. Employee #2 was partially trapped. He was being assisted by Employees #3 and #4 when a second cave-in occurred. Employee #2 suffered a sprained back and was hospitalized. Employee #3 sustained multiple bruises and contusions to his body and Employee #4 suffered bruises and contusions to his face; neither required hospitalization.
