EMPLOYEE KILLED WHEN STRUCK BY FALLING TUBES OF STEEL

AT APPROXIMATELY 9:40 AM ON JANUARY 3, 1992, EMPLOYEE #1 WAS OPERATING A VESCO 20 TON CRANE, S/N 91-153, COMPANY ID #9. HE WAS USING THE OVERHEAD CRANE TO MOVE LARGE, 40 FEET LONG, SQUARE TUBES OF STEEL. AS EMPLOYEE #1 WAS STACKING THESE BUNDLES, 3 TUBES FELL ONTO HIM AND KNOCKED HIM AGAINST OTHER STEEL BUNDLES. THE 3 PIECES WHICH FELL CAME TO REST ON TOP OF HIS LEGS. EMPLOYEE #1 WAS FOUND IN A SITTING POSITION WITH HIS BACK AGAINST A STACK OF STEEL TUBING AND BOTH LEGS PINNED BY THE 3 PIECES OF STEEL. THE WEIGHT OF THE STEEL (ONE TUBE WEIGHED 3,042 POUNDS) CAUSED FATAL CRUSHING INJURIES TO THE EMPLOYEE.
