EMPLOYEE LOSES TIP OF INDEX FINGER-OPERATING A PRESS MACHINE

At approximately 12:35 a.m., on July 26, 1989, Employee #1, a punch and stamp press machine operator, lost the tip of his left index finger while he was operating a Johnson Machine and Press Company Full Revolution Press. The press has Serial Number 56472. He apparently was operating the press without the use of restraints to prevent injuries while making punches.
