Investigation Summary

Investigation Nr: 980532
Event: 08/29/1989
EMPLOYEE LOSES LEFT THUMB-LEAD PLATE STACKING MACHINE

At 2:35 p.m., on August 28, 1989, Employee #1, a utility man and machine operator, lost his left thumb when he placed his left hand in the strap of the lead plate stacking machine. He was cleaning around the stacker machine. The two machines were connected by convey limit switches that keep stacks of lead plates in waiting to be placed in the machine. As he placed his left hand in the point of operation, the operator picked up a stack of lead plates. This caused the machine to start. He caught his left thumb on the machine. The employee's left thumb was amputated, and he was transported to the local hospital.
