Investigation Summary

Event: 06/16/1992
Employee's thumb & finger amputated by malfunctioning press

At about 9:00 a.m. on June 16, 1992, Employee #1 and a coworker were operating a full-revolution punch press. The press malfunctioned and the employee called maintenance. While maintenance tried to repair the press, Employee #1 was on the back side of the press feeding a piece into the press and removing a finished piece. The coworker was on the front side placing a cut piece to be finished into another slot in the die. Only the front side had operating palm buttons. For some reason the press tripped. Maybe the coworker hit the palm buttons or maintenance triggered it. The ram came down, went back up, hesitated a moment, then tripped again. Employee #1 reached in to retrieve the finished part before she saw the ram coming back down. Her right thumb and most of her index finger were amputated.
