Employee is killed in fall from 10 ft stepladder

Employee #1 was in the wrapper room near line 103 cleaning the ceiling, pipes and lights using a compressed air hose with a 24 in. adapter. When the air pressure was tested during inspection, it was at 108 psi. Employee #1, who was working from a 10 ft stepladder, apparently lost her balance or slipped and fell approximately 80 in. to the finished concrete floor. As she fell, she might have struck her head on the production platform near the point of work. She sustained crushing injuries to the side of head. The light fixture she was cleaning was displaced and had fallen to the ground, indicating that the employee may have reached for the fixture or struck it with her arm as she fell. There were no witnesses to the accident.
