EMPLOYEE KILLED WHEN TRUCK SLIPPED OFF JACK

At approximately 7:32 a.m. on July 29, 1989, Employee #1 was preparing to change the universal joints in the drive-line of an International-4200 dump truck. Employee #1 had jacked up the rear of the truck with a Walker-Model 193692 pneumatic jack and placed a jack stand under the left rear axle and was attempting to place a jack stand under the right rear axle. At that point, the truck slipped off of the jack. Employee #1 sustained traumatic head and neck injuries resulting in his death on July 30, 1989.
