EMPLOYEE RUN OVER BY TRUCK IS KILLED

At approximately 12:48 a.m. on November 14, 1989, Employee #1, a lagging supervisor, was walking near and on a roadway in a large foundation excavation site for a fifty-six-story office tower complex. A tandem dump truck backed over him, crushing him with both sets of right rear wheels. Employee #1 bleed to death thirty-six minutes later.
