Employee unties lanyard and falls to his death

At 11:45 a.m. on November 5, 1993, Employee #1 was one of two connectors receiving steel trusses. A truss was in place and was being held in place by the crane operator until Employee #1 was in place. The other connector was at his end ready to make the connection. Employee #1 had two lanyards. For an unknown reason, he untied one without first securing the other. According to an eyewitness he took one step and fell 6 ft 8 in. (18.8 m) to the ground; he was killed. Work had begun at 7:00 a.m. and was interrupted at 8:30 a.m. by a rain shower that lasted about 10 minutes. The iron workers had not gone back up on the structure until the supervisor and the union representative were satisfied that it was dry enough.
