Employee killed under lowered bucket

Employee #1, the operator, left his normal position at the control panel and went across a conveyor to investigate the reason a 35 cubic ft capacity bucket would not completely return to its resting position. His supervisor was operating the controls and raised and lowered the bucket once after its initial failure to seat. He raised it a third time, looked beneath it, and found Employee #1, who died of his injuries. The supervisor either did not realize the employee had gone to check out the problem, or he had operated the bucket without knowing exactly where the employee was.
