Investigation Summary

Investigation Nr: 170110886
Event: 02/04/1993
Employee's leg amputated after pinned by sliding dolly

On February 2, 1993, Employee #1 was standing adjacent to an opened overhead door waiting to close it as a series of four dollies cleared the doorway. As the fourth dolly, towed by a Bobcat, approached the doorway, the left front portion of the dolly struck a vertical post on the left. The impact forced the dolly to slide across the width of the opening, striking Employee #1's right calf and pinning his leg against the vertical post on the right. The severity of the injury to his calf muscles necessitated the amputation of Employee #1's right leg just below the knee.
