Employee falls from height and is killed

At approximately 5:30 p.m. on June 29, 2015, Employee #1 and a coworker were installing ceiling tile to a balcony on a third floor patio. The balcony was open sided with no guard rails. They were wearing a safety harnesses, but neither was tied off. Employee #1 was standing on a saw horse to elevate himself high enough to install the tiles. Employee #1 lost his balance and fell off of the saw horse and off of the balcony hitting the ground approximately 27 feet below. Employee #1 was killed.
