Event: 09/15/1993
Employee killed when struck by falling exhaust stack

A wheel-mounted crane was being used to reposition two exhaust stacks that had been removed from a vessel. The exhaust stacks were placed in proximity to each other in the shipyard while they were being prepared for sandblasting and painting. The crane operator placed stack #1 on its side and, as he attempted to place stack #2 on its side, he leaned it against stack #1. At this point Employee #1, a sandblaster/painter foreman, helped out by re-rigging the wire rope cable that held stack #2. The crane operator slackened the line holding stack #2 so that Employee #1 could reposition the wire rope cable. Employee #1 stood at the inside corner of stack #2. The 3 ton exhaust stack fell and struck Employee #1 in the back of the head, killing him instantly.
