Employee's Leg Ran over by Fork Truck

On June 7, 2004, Employee #1 jumped onto a step of a fork truck to ask the operator a question. The operator was unaware of the Employee #1 standing on the step and drove forward. Employee #1 was knocked off the fork truck and his left leg was run over. Employee #1 was hospitalized with contusions and abrasions of his left leg.
