Employee killed when pulled into dehydrator rollers

At approximately 2:30 a.m. on October 9, 1990, Employee #1 had been assisting another employee in the plant cleanup while the plant was shut down. Upon completion of this assignment, Employee #1 was directed to begin his regularly assigned cleanup jobs. Employee #1 had just come back from his break and climbed onto the drum deck. For some reason, the #3 dehydrator had been cleaned and inspected and the steam was on to heat it up to temperature, prior to the product starting through. It appears that Employee #1 reached into the roller area beyond the safety bar and one of his hands was caught. He placed his other hand in the same area to free himself. Employee #1 was drawn into the roller up to his shoulders. He died of a broken neck and some internal injuries. The employer was issued a citation for not having a lockout/tagout program and for inadequate machine guarding.
