Employee dies of asphyxia in cave-in

Employee #1 was struck from behind by a cave-in and buried with his head in the water. When his head was pulled from the water, he was breathing and talking, but then a second cave-in buried him and two rescuers. The two rescuers were saved, but Employee #1 died of asphyxia.
