Employee killed in fall from roof

At approximately 8:37 a.m. on May 18, 1995, Employee #1 was approximately 45 ft above ground level on a roof helping a coworker place trim on a chimney. Employee #1 was on a small 5:12 pitch inclined roof with a 2 by 4, holding the trim while the coworker tacked the board. The coworker heard a yell and saw Employee #1 hit the rail on a balcony and fly over the top; he did not see the actual fall. Another coworker saw Employee #1 hit the metal rail on the first floor and land on the ground. Employee #1 died in the hospital from massive internal injuries.
