Investigation Summary
At approximately 10:00 a.m. on November 16, 1992, Employee #1 was nailing a 4 by 8 section of particle board to the roof of a residential building under construction. Apparently the board slipped, and Employee #1 lost his balance and fell 17 ft 10 in. to the ground. He was killed.
