Employee killed in fall from ladder

At approximately 3:15 p.m. on May 12, 1995, Employee #1 was asked by his supervisor to fix a piece of equipment. Employee #1, in turn, asked a fellow coworker to repair a crane. When Employee #1 finished the requested repair, he asked if the coworker needed any help with the crane. The coworker said no, but Employee #1 started up the ladder to the coworker anyway. He had gotten approximately 2/3 of the way up the ladder when he slipped and fell, hitting an electrical wire that led to the water cooler. The electrical wire caught him under the throat, rotating his body 180 degrees. Employee #1 continued falling, hitting two other coworkers, then hitting the corner of the water cooler before landing on the floor. Employee #1 died.
