Investigation Summary
Employee #1 was one of three workers on a 5:12 pitched roof deck. The workers, who were preparing to erect a window dormer on the side of the main roof, were gathering tools and laying chalk lines. Employee #1 stepped on a roof section that was covered with a light layer of frost. He lost his footing and fell over the roof edge, striking his head against the frozen ground when he landed. He was killed. The roof eave was 33 ft above the ground. No fall protection system was being used.
