Employee killed in trench cave-in

Employee #1 was preparing a gravel bed in a trench to lay a 20 ft long by 8 in. diameter pipe. The trench was approximately 22 ft long by 19 ft deep by 35 ft wide at the top. It angled down steeply to straight vertical and was 40 in. wide for approximately the last 10 ft to the bottom. Employee #1 was standing in the trench signaling for gravel when the south wall of the trench caved in, burying him. He was killed.
