Employee crushed and killed under falling truck

At approximately 10:00 a.m. on May 5, 1990, Employee #1 was removing a gas tank from beneath the rear end of a GMC Suburban. He used a bumper jack to lift the truck into position and, without cribbing, blocking, or securing the truck, he crawled underneath and began removing the gas tank. The jack slipped, causing the truck to fall on Employee #1. He was crushed between the rear end of the truck and the hood cover lying on the ground. Employee #1 suffered multiple injuries to his head and upper body, and died at the site.
