Investigation Summary
Employee #1 and three coworkers were involved in detail roofing work on a barrel-type roof that measured 22 ft at its crown and 16 ft 4 in. at its lowest point. Employee #1 stepped backward off the peak of the roof, falling onto a parked pickup truck and then onto the asphalt parking lot. He was killed. The employees had not been provided fall protection equipment.
