Investigation Summary
On September 12, 1992, Employee #1 was driving a skid steer front end loader. Apparently, he left his glove(s) on top of the loader's cab. Without dismounting or shutting off the vehicle, he apparently climbed in front of the cab and accidentally stepped on the foot pedals that activated the boom/bucket. The boom rose and Employee #1's midsection was caught between a portion of the boom and the top front of the loader's cab. He sustained liver damage, but was able to dismount the loader, go to another room, and relay the accident to coworkers. Employee #1 was taken to a local hospital, where he underwent exploratory surgery. The next day, he died en route to a hospital in Wichita, KS.
