Investigation Summary

Investigation Nr: 565747
Event: 09/24/1990
Employee killed when run over by trash truck

Three employees of Arlington Disposal Co., Inc., the truck driver and two pickup men, were picking up residential refuse. Employee #1, a pickup man, was on the right side and the other pickup man was on the left side. The driver observed both pickup men on the platform and turned his attention to his driving. After making a right turn and traveling a short block, the driver checked the rear view mirror. Because he didn't see Employee #1, the driver stopped the vehicle and told the coworker to go back and see where Employee #1 was. Thinking a bag of trash had fallen off the truck, as it does occasionally, he started to back the truck until the coworker ran up to the truck and said that Employee #1 had been run over. The coworker ran to a house and asked the resident to call for an ambulance. Employee #1 lost either his footing or his handhold, or both, and fell under the rear tandem wheels of the vehicle. His upper body and head were crushed. Employee #1 was killed. It is believed that he was eating while working, which is against company policy.
