Employee dies in fall from roof

At approximately 8:00 a.m. on July 24, 1996, Employee #1, the lead man, was separating sections of roof decking into two separate bundles. He was working near the northwest corner of the 129 ft long by 137 ft wide by 24 ft high prefabricated facility that was under construction. Employee #1 fell 22 ft 7 in. through the purlins supporting both bundles of roof decking and landed on the concrete floor. He suffered massive head injuries and was transported to the hospital, where he died at approximately 10:00 a.m. on July 25, 1996. Employee #1 was not using any form of conventional fall protection.
